2009 Gaming Per Capita Schedule
If you HAVE NOT moved or DO NOT HAVE a name change, DO NOT submit any forms.
If you have moved or have a name change, your W-9 and notarized Change of Information
and any additional documents must be in the Per Capita Office by close of business on the
last day that forms are accepted for each first check run.
All checks will be mailed to the W-9 address. Checks will not be held or handed out.
Please mail your forms as soon as possible.
- Friday, July 3rd, 2009
- Enrollment's adult membership roll finalized for Gaming Per Capita issuance.
- Friday, July 31st, 2009
- Last day to submit forms for first check run.
- Forms must be in Per Capita Office by close of business in order for your check to be printed in the first run.
- August 1st through August 21st
- Per Capita Office closed except for appointments during this time.
- Appointments can be made by calling Per Capita every morning between 8 AM to 9 AM. Please know your roll number.
- Appointments are for the receipt of 2008 Gaming per capita, delivery of a police report, proof of filing a court order in the Cheyenne and Arapaho Tribal Court, or a Cheyenne and Arapaho Tribal court order.
- Messages will be answered in the order received and as time allows.

- Monday, August 17th, 2009
- Gaming checks printed (August 15th falls on a Saturday)
- Monday, August 24th, 2009
- Per Capita Office is open
